Food vendors: submitting your health permit application online
The Town of Yarmouth Health Department prefers to have food vendors submit their health permit application online.
Below are some easy to follow instructions for submitting the online application. Please note, hyperlinked text will bring you directly to the page being referenced but we are also providing complete instructions.
Visit the Town website and click on “Online Services” beneath the image area with search function.
Scroll down to the “PERMITS & LICENSES” heading and click the button to access the OpenGov Permits and Licenses site.
Click the Explore button under Health to access the online payment portal and then click the Select button to submit a food establishment application.
After reading the Food Establishment Application guidelines, click the Apply Online button.
Fill in the various form fields as requested.
Note: On Step 7 of 12 you will need to select Temporary Food for Establishment Type in order to request the $15 permit needed for the Yarmouth Seaside Festival. All other form fields are specific to your business and the area you serve.
